The Healthcare Forms Workshop educates participants on the following forms: Healthcare Power of Attorney, Advance Directive and HIPPA privacy. This event is designed for adults 18+.  The legal forms will be provided for you and an attorney will be present to explain each document. A notary will attend as well.
